Conservative Dentistry with CERASMART "no prep" veneers

I love CERASMART for these applications. Here is a 20 year old who came in post ortho and post previous bonding that he had done when he was 14 year old. The bonding was starting to breakdown and I have no doubt it was because the spacing was so large. I hate to cut down teeth if I don't have to and wanted to be really conservative so I decided to so a prepless veneer.

Loving those thin margins!!!

(Inspired by Yao-Lin with my pictures. I still need practice!)

There are some compromises on a case like this but I can live with it. Yes, #9 is a little too wide compared to #8 but the space on the left side was a lot larger so I had to hide it somewhere. I don't love that the laterals are a little thick and facially inclined. I would have loved to tuck them back in lingually a touch, but with his bite on the left side this was tricky and again, without prepping for space it is what I gave up. adding some texture and composite colors really add some fun layers to these cases. He was really happy. His gums no longer bleed and they feel more stable to him. He was most excited with the fact that he didn't have to get numb!!!!

Kristine,

I have two cases like this in planning. Can you send me your rst files so I can see the preps?
It seems that you would atleast have to round the line angles on a normal case. Yours it seems once you took off the old composite you had what you needed. I may have to get a wax up for mine since I don't have much to start with.
Lastly, would it be easier to use the colors from gc or will they wear more quickly? Thanks.

Jeff

Kristine,

I love your conservative way of approaching the peg laterals.

I have a very similar case that is coming up Monday but more complicated.

I am going with non-prep or very minimally prep too for my next Monday case and will be using GC Cerasmart too.

Jeff- Happy to send you the rst file tomorrow when I'm back in the office.

As far as GC vs Cosmedent colors, I haven't used the GC before so I'm not sure. I've found with the Cosmedent, as long as I add a thin layer ofcomposite over the top and seal in the color it will stay vs just light curing color over the top alone.

Post your case!! Can't wait to see it!

Yao-Lin I can't wait to see your case and even more to see your photos!! Every time I see one of your posts it just makes me want to work harder on my photography and presentation!

Great work Kris!  Tough case with the large spaces post ortho.  Patient must be thrilled!

Fantastic result as always.  Do you have any concerns about the CeraSmart debonding?

Did you request the peg laterals to be placed in the middle of the space, or did the ortho just do that because of the previous bonding?  I always request them to be centered to allow this type of no-prep approach to be used.

Why CERASMART over other material options, other than the ability to tint the surface with composite modifiers?

Thanks for sharing!

Hi Ernie!

This patient  came to me post ortho so I didn't get the option of where to place the laterals but I do agree with you. I like them either centered or a little mesial so if there is a large discrepancy I can tuck it in on the distal end.

As far as CERASMART, I like it because it is so forgiving to place with the no prep and the margins mill out beautifully. This pretty much snapped into place. If I would have tried that with e.max that snap would have brought me to tears!

Thanks Dr. Chuck!

I haven't had any problems so far with debonding. Especially on cases like these. I do cement with a resin cement and then add a thin layer of composite on top of that so if feels pretty sturdy at least in my head ;)

The good news is, if it does debond, it's annoying but nothing fatal because there is no exposed dentin. Just a peg lateral that doesn't look as esthetic!
